This Event Request Form is required to obtain permission to have any new and/or non-routine, non-recurring events in the synagogue or to have a synagogue or synagogue sponsored event outside the synagogue.  Please use the note section below if needed.  Submission of this form does not guarantee approval of the event. Please fill out the room reservation form in as much detail as possible, this will help us determine the availability of synagogue resources as well as aid us in calculating any charges related to the event requested.

This form should be submitted at least 90 days prior to the requested event/activity date.

Final approval may be subject to the Rodeph Sholom Programming Committee
